This looks massively photoshopped. The head on the person on the right does not look like it belongs at all.

I don't know what you mean with "does not look like it belongs at all". Is it just the hairstyle that looks "modern" maybe?
It is the lighting from her neck up. Perhaps that is a style from the photographer, but it looks almost like her head was photo on to me.
